		Precise Support Assist-Life Stage’s Transition service provides short- and long-term supports to help participants live at home and participate in their community. Our experienced and skilled staff can work with participants and their families to build daily, weekly, and longer-term support schedules, ensuring that their supports help them reach their goals at all times. we can also help with budgeting, including spending planning, one-time purchases, and monthly payments
we excel at helping NDIS members through life stage transitions. Beginning a new school year, moving out on your own, landing one’s first job, and finally reaching retirement age; with the right planning and networking, we can help you get through the transitions in your life and reach your goals.

- Life planning and goal planning
- Financial mentoring
- Daily planning and Budgeting
- Establish supportive networks
- Capacity building skills
- Development of independent self-care and living skills
- Linkage support Mainstream supports
- C Assistance with decision making
- Coordination of supports
- Crisis and conflict resolution
- Assistance with accommodation and tenancy obligations
